Employees Head Crushed Between Fork Lift and Wood Block

At approximately 3:30 p.m. on April 20, 2005, Employee #1 was working inside a service pit, 6-ft deep by 17-ft long and 3-ft wide. He was repairing a fork-lift, Yale Number 305. He was tightening a bolt on the crank shaft shield of the fork-lift while standing on a metal bench inside the service pit. His head was sticking up through the opening between the fork-lift and the wooden blocks used to cover the unused portion of the service pit. Another fork-lift operator drove his fork-lift into the service area and parked near the service pit behind the fork-lift Employee #1 was working on. The other fork-lift moved forward and pushed the wooden blocks, pinning Employee #1's head between the fork-lift and the wooden blocks. Employee #1 received a skull fracture and was hospitalized.
