Investigation Summary
At 7:45 a.m., on April 5, 2005, Employee #1, a machine operator employed by Villa Roma Sausages, Ontario, CA, was working at the sausage-stuffing machine, loading the sheep casing onto a three prong mechanical component. At the end of the cycle, Employee #1 climbed a portable six-foot aluminum ladder to reach the top of the machine hopper in order to clean the machine before the next cycle run. He stated that the machine was not running at this time. He was standing on the third rung of the ladder, pushing the residue of the sausage mixture through an opening at the bottom of the hopper using his right hand. The usual procedure was to use a long metal tool to push the sausage product through the opening at the bottom of the hopper, but he could not find the tool, and proceeded to use his hand. As he reached into the hopper, he felt his fingers being cut by the blades beneath the opening. He sustained right middle and ring finger amputations, but was not hospitalized for the injuries.
