Investigation Summary
At approximately 9:15 a.m. on January 20, 2005, Employee #1, a laborer, was instructed to drive an International Truck, Number 490, to the field to unload a roll of plastic, 13-ft long and weighing approximately 260 pounds. At the field he climbed up the deck of the International Truck, which was 14-ft long by 8-ft wide and 10.75-ft high. He began to untie the plastic rolls and move them to the edge of the platform to push them off to the ground. As he was untying a plastic roll, he fell from the platform to the ground. Employee #1 was not wearing any personal fall protection and was hospitalized with brain damage.
