Employee Crushes Hand in Chain Conveyor Pulley

At approximately 6:00 p.m. on January 28, 2005, Employee #1 was operating a concrete packaging machine (Bemis Packaging Machinery Company, Serial Number 5521C34, and Model Number 1). While operating the machine, Employee #1 saw a broken bag of grout on the conveyor. As Employee #1 tried to pick the broken bag up, his left hand got caught in the pinch-point between the end pulley of the chain conveyor and the transfer roll and caused a crush (avulsion) injury to his left hand which required a surgery and hospitalization for six days.
