Investigation Summary
On October 21, 2004, Employee #1, who worked for an electrical contractor, was using a 10 ft tall A-frame fiberglass ladder to attach conduit along the wall of a school gym in Loma Linda, CA. He was standing on approximately the third rung from the top of the ladder, which was closed and leaning against a wall. Employee #1 had finished drilling a hole for an anchor and was handing the drill to a coworker when the ladder slid down, struck the ledge of a ticket window in the wall, then bounced onto the wooden gym floor. Employee #1 landed feet first, sustaining a plateau fracture of his left tibia. He was transported to Loma Linda Medical Center, where he underwent surgery and was hospitalized for three days. The floor had recently been waxed. Employee #1 later stated that he had checked the ladder before using it, and that he thought it was in a safe position.
