Employee's Finger Is Amputated in Threading Machine

At approximately 2:10 p.m. on September 24, 2004, Employee #1 was operating a threading machine, a Wheeler Rex, Model Number 6790, Serial Number 784517, to thread a 0.75-in. schedule-80, hydraulic pipe. A coworker was helping him and they had made four threads on two pipes. Employee #1 fed a pipe into the machine but the pipe was not locked. He was trying to close the jaws with his left hand and was holding the pipe with his right hand. He pushed the foot pedal switch with his right foot and the pipe became stuck between the pipe and threading die. His right smallest finger was crushed between the pipe and threading die. His finger was amputated at the hospital.
