Investigation Summary
At approximately 3:00 p.m. on July 13, 2004, Employee #1, a regular employee of Dessert Seed LLC, was operating an LP50 Optimum Nissan forklift, Model Number PJ02A25PV, unloading pallets of onions. While operating the unloaded forklift, it hit a dirt creek causing it to tilt. The employee became scared and jumped off the forklift. When he jumped from the forklift, he became pinned between the forklift and a shed. His right elbow was crushed and he was hospitalized for two days.
