Investigation Summary
At 9:20 a.m. on September 1, 2004, an employee worked for a company that grew, processed and packed carrots. Employees operated a variety of machinery that cut off the tops, washed, sorted and bagged the carrots. Employees #1 and #2 were repairing a dump bin. Employee #2 raised the dump bin and crawled underneath it to adjust a brake chain, which was under tension. Employee #1 stood in front of the frame and looked down at Employee #2. Employee #2 took off the master link on the brake chain, and then removed the chain. The dump bin fell, and struck Employee #1 on the head, crushing him. Neither employee followed the employer's lockout-tagout procedures to prevent inadvertent movement of the bin. Employee #1 died from massive head trauma. Employee #2 was hospitalized for fractured ribs and left hemopneumothorax. The steel dump bin weighed approximately 600 lbs and was mounted on a 58-in.-wide by 44-in.-long by 40-in.-high frame. The bin was powered by a 2 hp U.S. Electric Motor and equipped with a skip stop electric brake. A chain and sprocket configuration was attached from the electric motor to the bucket of the bin in order for the bucket to move up and down. Both chains had a master link for adjusting them. The motor, sprocket, and chain drive system were located underneath the bin and guarded.
