Investigation Summary
On July 2, 2004, an employee was working in Bay 3 cutting signode bands that were securing a four bundle load of TS 10- by 6- by 500- by 30-in tubing. Three pairs of signode bands were securing the load. A hand-held cutting tool was used to cut the metal bands. The load that the employee was preparing to cut the bands on, was stacked on top of a four load bundle of TS 10- by 6- by 500- by 32-tubing. The employee was standing on the ground in between two stacks of secured tubing. Another stack of secured tubing bundles were located immediately adjacent to the stack of tubing where the employee was preparing to cut the bands. The top of the stack was approximately at the employee's knee level. When the employee snapped the last pair of three bands, two of the four tube bundles shifted and landed on the employee's lower right leg. The employee was hospitalized and treated for a leg fracture. The employer stated that the employee had been taught to stand on the adjacent, secured, load and cut the bands from the top of the stack.
