Employee amputates finger in pamphlet insert machine

At approximately 2:20 p.m. on May 7, 2004, an employee was troubleshooting a GMS pamphlet insert machine, GMA SLS-1000. He was inspecting a belt and pulley drive assembly. When he reached into the nip- point of the 1-in.-wide flat belt and pulley assembly with his right hand, first digit of his right index finger was amputated.
