Employee killed when head Caught in concrete block machine

On October 29, 2004, Employee #1 was not performing his regular job duties of being a shift supervisor but that of a concrete block machine operator. The normal operator was out sick and Employee #1 had operated the machine before. Employee #1 went inside the provided guards in order to clean the area and his head was caught between the frame and the transporter. He received severe head trauma which caused his death.
