Investigation Summary
On April 19, 2004, Employee #1, a truck driver, walled between the logging truck and a Prentice loader. The limb gate was being loaded onto the truck at the time. The limb gate fell striking Employee #1 and knocking him head first into frame of the trailer. He sustained massive head trauma and was pronounced dead at the scene.
