Investigation Summary
At approximately 2:00 p.m. on July 27, 2006, Driver #1 was operating a dump truck loaded with dirt. Driver #1 was stopped in a driveway area, waiting to empty her load. Driver #1 was told by the job site superintendent, who was in the same area, to dump her load. Driver #1 pulled forward and turned ninety degrees to the south to line up the back of the dump truck with the area in which the dirt would be dumped. Driver #1 then observed the signal man signal her to back up. Driver #1 checked her left side mirror and did not see anyone, so driver #1 looked in her right side mirror again and saw the signal man and began backing up, keeping her eyes on the signal man. Driver #2 saw Driver #1 start to back up and back over Employee #1. Driver #2 then started to blow the truck's horn to get Driver #1's attention. When Driver #1 looked up, Driver #2 pointed to Employee #1 on the ground. Employee #1 died from the injuries he sustained.
