Investigation Summary
On August 25, 2005, Employee #1 and Employee #2 were in a basket that was being lifted into the air by a rough terrain forklift. The employees were trying to install eve purlins. An eve purlin approximately 25-ft long was laid across the guardrails of the basket and lifted at the same time with the employees. The forklift lifted the employees and the purlin up to the South end attachment point. The employees began to slide the purlin toward the North to the other attachment point. As they did this, the basket flipped off of the forks, throwing the two employees to the ground. The basket fell on top of Employee #1, killing him. Employee #2 sustained a multiple fractures, and was hospitalized.
