Employee Falls Off Ladder and Dies

On March 2, 2005, an employee was using a 6-ft stepladder to access a storage area in order to retrieve flexible ductwork. The storage area was located approximately seven feet above the ground. The employee was using a ladder that had a missing footpad making it unstable. When the employee was removing flexible duct from the shelf, he lost his balance and fell off the ladder striking his head on the concrete floor. He died approximately 10 days later due to his injuries.
