Employee Dies in Fall from Scaffold

On January 4, 2005, Employee #1 and coworkers erected a job-made wooden scaffold inside of a single family residential home to set the rafters. The employee was climbing the wooden members of the scaffold when he slipped and fell approximately 18 ft. Employee #1 was transported to the local hospital, where he died at 6:21 a.m. on January 5, 2005.
