Investigation Summary
On March 28, 2008, Employee #1 and a mechanic were installing two travelers for an elevator. Employee #1 was working in a hoist way which houses two cars. Employee #1 and the mechanic were on top of a car, level with the 4th floor landing. They were making connections of the conductors onto the terminals inside of a junction box. The two travelers had approximately 87 conductors. As Employee #1 was making his 20-plus connections into the junction box, he placed his right foot onto a 3-in. wide spreader beam. The spreader beam divided the hoistway between the two cars and was also known as a divider beam. The adjacent car in the hoistway was traveling downwards and the end of its header pinched Employee #1's toes against the spreader beam. Employee #1 was taken to a hospital and treated and released for bruises and a compound fracture on his fourth toe.
