Employee's Fingertip Is Amputated When Caught in Lift Gate

At approximately 7:40 a.m. on February 25, 2008, Employee #1 was working for a hospital and was delivering medical supplies. He was standing on the ground outside the passenger side of the truck towards the rear. The lift-gate control switch to open and close the lift gate was located on the outside of the box of the truck. He pushed the switch to open the lift gate, and he noticed that the exterior chain, which supports the lift gate on the driver's side, was tangled around the lift-gate support bar. He pushed the switch to close the lift and walked over to the driver's side, where the lift gate chain was tangled. He reached up with his right-hand and grabbed the chain, trying to loosen it with his fingers wrapped around it. As he was trying to loosen the chain, the lift gate all of a sudden opened and the chain grabbed his finger. Part of his right index finger was amputated, and he was hospitalized.
