Investigation Summary

Investigation Nr: 201184207
Event: 02/14/2008
Employee Suffers Concussion in Fall from Boom Lacing

At approximately 7:30 a.m. on February 14, 2008, Employee #1, a Foreman, was assigned to work with a crew that was taking a 150 ton crane, Model Number EO00861, apart to ship to a job site. Employee #1 was standing on the lower section of the crane boom lacing that was about 53 inches off the ground. As he was lifting up the pendant line to take the pin out of the boom, he slipped and fell backwards. Employee #1 landed on his back, striking his head and neck. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treaded for a concussion. The accident investigation revealed that Employee #1's left foot had slipped off the thin, cylindrical tube of the boom lacing, which caused him to lose his balance and fall. He should have used a ladder to access the crane instead. The employer was cited under Title 8 California Code of Regulation Section 1675(a) Ladder general requirements.
