Investigation Summary
On September 26, 2007, Employee #1 was working as a laborer. Employee #1 was to remove a window from its frame and take it to the debris box for disposal. Employee #1 was wearing a pair of gloves. Employee #1 should have been provided with a longer pair of gloves to protect the forearm while he was removing the window. According to the general manager, the incident took place during break time, and no one had knowledge that Employee #1 was still trying to remove the window. At approximately 10:30 a.m., Employee #1 lacerated his right forearm. The foreman drove Employee #1 to the hospital for treatment. Employee #1 was treated for a 2-in. laceration and released after three hours. Employee #1 was then transferred to another hospital where he was hospitalized for three days.
