Investigation Summary
On June 18, 2007, Employee #1 was working for Lisa Cheng Ng dba Mongkok Dim Sum & Restaurant as a kitchen helper located at 2438 Noriega Street, San Francisco. The employer specializes in Chinese food and dim sum. The supervisor was in the kitchen. Employee #1 had finished cleaning the 1h.p. table top "Somerset" dough brake in the kitchen and had just started to operate it. While operating it, the cover for guarding the roller was still removed. The cover was not returned and secured back into position. Employee #1 began to feed the dough into the dough brake. At approximately 9:20 a.m. Employee #1's right four fingers were placed into and under the moving roller. Employee #1 stopped the dough brake as coworkers came to help. The emergency services were called. Employee #1 was taken to Ralph K Davies Hospital. The fourth finger was surgically operated to treat the crushed tip and damaged tendons. Skin grafting was performed on all four fingers. The kitchen supervisor and Employee #1 were interviewed during the course of the investigation. Employee #1 had finished cleaning the dough brake and had forgotten to replace the cover to guard the roller while operating it.
