Employee Is Killed in Car Accident

At approximately 6:30 a.m. on July 30, 2007, Employee #1 was operating truck Number 15138. This vehicle has dual steering positions and can be operated from both sides of the cab compartment. The right side of the cab compartment door was secured open and that there was no damage to that door. Employee #1, the driver, was operating the vehicle with the right side cab compartment door open. It appeared that Employee #1 was not wearing a seat belt at the time of collision. Due to the damage to the right side cab compartment, authorities were unable to determine if there were any mechanical defects at the time of this collision. The subject vehicle was towed for a mechanical inspection. Due to street cleaning day, there were no parked vehicles on the south side of the street (accident side). The truck stopped eastbound on the street, headed eastbound on the sidewalk of the street and the right front end made contact with the stationary utility pole. The force of impact broke approximately a ten-foot section of the utility pole. The truck continued eastbound on the street with the right side contacted the remaining suspended portion of the utility pole. Employee #1 was partially ejected from the cab compartment and was projected to the pavement. There was no brake marks observed on the pavement. The driver was pronounced dead at the hospital at 8:20 a.m. on July 30, 2007.
