Investigation Summary
At 10:45 p.m. on Tuesday, June 7, 2005, Employee #1 was working as a bartender. He was shaking a martini in a Boston shaker. On the top of the shaker was a pint glass 6 in. high and 3.5 in. in diameter at the top, with the open end down. The Boston shaker itself was a stainless steel shaker 7 in. high and 3.625 in. in diameter at the top, with the open end up. When the top and bottom are sealed together, the Boston shaker measures 11.5 in. high, with the inverted glass inside the stainless steel bottom. To release the seal, the bartender ordinarily bumps the stainless steel shaker right where the glass's top rim is positioned. When Employee #1 bumped the Boston shaker on the counter, the glass shattered and lacerated his wrist. Emergency medical services were called. Paramedics provided emergency care and transported Employee #1 to California Pacific Medical Center. Employee #1 was hospitalized on Wednesday, June 8, 2005, at 2:15 a.m.. He underwent surgery. He was released on Saturday, June 11, 2005 at 1:38 p.m.
