Employee Falls from Rebar and Sustains Fractures

At approximately 08:40 a.m. on June 14, 2005, Employee #1 was working as a lead carpenter. Employee #1 was performing rebar work and was traveling from point to point. Employee #1 was climbing from the inside of the wall panel to get out and over the form. Employee #1 was wearing a harness and positioning device, but was not tied-off. Employee #1 fell approximately 18 ft to the plywood flooring. Employee #1 sustained a fractured left shoulder, wrist and ribs and was hospitalized for less than 24 hours.
