Investigation Summary
At approximately 3:30 p.m. on June 24, 2005, an employee, an experience carpenter, was working with a miter saw. As he was relocating the saw section, he planned to place the saw on the floor before moving the table. The saw blade was locked in place and it was in down position. However, the employee did not unplug the saw from power source. As he was picking it up, he placed his right hand on the handle and with left hand he was supporting the base of the saw. The cotter pin, which locks the handle, for some reason, caused the handle assembly to suddenly open. The weight of the saw shifted, his left hand slipped out from under the base and went towards the saw exposed blade and by pressure of his right hand as the handle rolled fired the power switch. As a result, he completely severed the last joint of his pinky and his ring finger was severely cut but dangling from his hand. The employee was hospitalized and treated for amputation of fingers.
