Investigation Summary
At approximately 3:45 p.m. on November 3, 2004, Employee #1, the vice president of the company, was assisting with the unloading of a scissor lift from a flat bed truck by using a forklift operated by another employee of the company. When the fork lift operator had the scissor lift up, he requested the truck be moved forward to provide more space to maneuver. The truck driver refused to do so. The forklift driver began to turn slowly toward the curb side. Due to slight slope of the street, the scissor lift began to tilt. Employee #1 tried to hold the top portion of the load. The load was too heavy to support and it came down on him slowly. Employee #1 suffered injuries to his lower torso and right leg (contusion) and was hospitalized.
