Investigation Summary
At about 11 a.m. on October 25, 2004, Employee #1, an electrician for the City and County of San Francisco, California arrived at the unit Number 912 to install a vent in a new cabinet above the stove. Approximately six months earlier, a stove fire burnt the vent. The tenant stated that Employee #1 went out for parts and returned about 11:15 a.m. Again, he went out and returned about 11:45 a.m. The tenant heard sawing, possibly to widen the cabinet hole to accommodate a 5 in. in diameter pipe duct. About 1:00 p.m. the tenant heard a crashing sound with a metal noise. A short time after 2:00 p.m., she went to the kitchen and saw Employee #1 lying on the floor in a fetal position. Employee #1 was facing the exterior hall wall with his head near the door and his feet towards the sink and stove. When the tenant touched Employee #1, he was cold. Then she called for an ambulance. The paramedics worked on Employee #1 for about one hour without success. A medical examiner arrived and transported the body to the medical examiner's facility. The cause of Employee #1 death was an acute myocardial infarct due to acute coronary thrombosis, the result of atherosclerotic coronary artery disease.
