Investigation Summary
At approximately 11:20 a.m. on December 29, 2003, Employees #1, #2 and #3 were working from a pump jack scaffold to construct a brick exterior wall on a single family home. The scaffold included seven pump jack poles made from 2-ft by 4-ft lumber. Employee #2 and #3 were working between the fifth and sixth pole and Employee #1 was working between the sixth and seventh pole when the scaffold collapsed. Employees #1, #2 and the coworker fell to the ground when the scaffolding collapsed. Employee #1 received fractures to his leg and ankle, and a laceration to his head. Employee #2 received fractures to his arm and ribcage. Employee #3 refused treatment from medical personnel on site. Employees #1 and #2 were hospitalized and treated for their injuries. The sixth pole was observed to have broken in two places beneath the pump jack at the splices of the 2-ft by 4-ft lumber. Mending plates were not installed at the 2-ft by 4-ft splices of the sixth pole, which was broken, to develop the full strength of the members. The scaffold platform where the employees and the coworker were working from was measured at 17-ft, 2-in. above the ground.
