Investigation Summary
At approximately 10:00 a.m. on March 24, 2004, Employee #1, a coworker and a foreman were performing roofing operations on the new addition to the Harley Davidson building in Fairfax, VA. The foreman instructed the two employees to remove a 4-ft by 4-ft skylight, placed below them on the ceiling, in order to install rolled rubber roofing material on the new roof. Employee #1 and his coworker removed and lifted the skylight through a roof opening. At this time, when Employee #1 was raising the skylight, he stepped forward into the opening and fell 15 ft to a concrete floor. He was transported to a local hospital, where he was treated for head and back injuries. On March 26, 2004, Employee #1 was declared brain dead and was removed from life support systems. The foreman was supervising the task at the time of the incident.
