Employee Is Killed When Pushed from Truck by Material

On June 22, 2005, Employee #1 was standing on the flatbed of a delivery truck pulling materials off the blades of a Hyster forklift that was raised and tilted forward. The materials slid forward, striking him in the chest, and causing him to fall off the truck onto the concrete floor. He struck his head against the floor, and he was killed.
