Investigation Summary

Investigation Nr: 200202372
Event: 06/23/2004
Employee killed by fall after receiving electric shock

On June 23, 2004, Employee #1 was told by a supervisor to replace 13 light fixtures in a building. He replaced 10 of the fixtures and returned the following day to finish the job. He was using a 6 ft tall aluminum ladder when he contacted an energized wire and received an electric shock that caused him to fall. His head struck the floor as he landed, and he was killed. Employee #1 did not de-energize the circuit at the breaker on the first day, but did turn off the light switch. On the second day, he did not make sure the light switch was turned off.
