Investigation Summary
An employee was working on a part for a bull dozer. He was repairing a broken spring and cylinder. Both the spring and cylinder were in a hydraulic press. The press failed which caused the spring and cylinder to dislodge and strike the employee in the face. He died as a result of his injuries.
