Investigation Summary
On March 29, 2004, an employee was assisting in a six-person landscaping operation, which consisted of blowing mulch over a previously seeded area without any means of communication or fall protection The employee fell off the back of a moving truck loaded with bales of straw, was struck by, ran over and killed by the straw mulching blower equipment, which was being pulled forward by the truck.
