Investigation Summary
On February 2, 2006, Employee #1 was standing on a 10-ft metal stepladder and hammering a nail into a wooden frame structure without wearing safety goggles. In this process, the nail ricocheted and struck Employee #1's right eye. Employee #1 sustained a rupture of the right eye globe and was hospitalized until February 6, 2006.
