Investigation Summary
On December 20, 2005, as per the instructions of the company owner, Employee #1 loaded three truck chassis onto a Freightliner trailer. Since these chassis were required to be secured to the trailer, Employee #1 climbed on the trailer while holding the securing chain in his left hand. While standing on the trailer, he placed his right foot on the first chassis then, placed his left foot on an axle. The axle rotated in the clockwise direction. Employee # 1 lost his balance and fell approximately 6 ft to grade. Employee #1 was admitted to Highland hospital for treatment of a fractured left femur.
