Employee Amputates Finger Cleaning Food Preparation Machine

On October 4, 2005, Employee #1 was working for a manufacturer of prepackaged meals, cleaning machines used in food preparation. Specifically, he was cleaning the divider/rounder machine, which stretches dough. To clean the machine, it was his practice to turn it on to advance the rolls. While the machine was energized, he would open the interlocked gate, stopping the machine. At that time, he would reach into the machine and clean it. Employee #1 suspects that, when the accident occurred, his elbow pressed on the interlock safety switch, and since the machine was energized, the rolls started rotating. The middle finger of his left hand was caught, and it was partially amputated. Employee #1 was transported to San Leandro Hospital, where he was treated and released. The injury line on the report says he was hospitalized. A special lock is needed to lock out the machine, but it was not available. Employee #1 was not given extension tools to perform the cleaning while the machine was energized. The supervisor did not tell him how to clean the divider/rounder machine; Employee #1 learned by watching other employees. They cleaned the machine while it was energized. Employee #1's training involved watching a lockout/tagout video about machines that were different from those he cleaned, and there was no instructor to answer questions. Causal factors have been established. Employee #1 was cleaning the divider/rounder machine without de-energizing it or using extension tools to keep him away from the pinch point. He was not provided with locks, tags or extension tools. He was not trained in the control of hazardous energy for the cleaning of the divider/rounder. There were no separate, written procedural steps for the control of hazardous energy for the divider/rounder.
