Investigation Summary

Investigation Nr: 201506326
Event: 08/26/2005
Employee's Foot Is Fractured When Run over by Truck Tire

At approximately 7:15 a.m. on August 26, 2005, Employee #1, working for Fidelity Roof Company, was walking in the company's service yard. He asked a coworker to move a service truck that was parked in back of the service truck he wanted to use. The coworker got into the vehicle and looked in his right side-view mirror, preparing to turn the rear of the truck to his right. He did not see Employee #1 in the mirror. As the coworker was backing the service truck, Employee #1 walked on the left side of the moving truck, between the moving truck and the parked truck. The coworker was looking in the right side-view mirror and did not see him in his path. The left front tire of the truck struck Employee #1's left foot and ankle. The impact of the tire fractured his foot and ankle. He was hospitalized for his injuries.
