Investigation Summary
On August 9, 2005, Employee #1, a truck driver, was redistributing a load of chemicals that were located in the back of the tractor-trailer truck he was driving. The trailer was equipped with a hydraulic lift gate that, in the closed position, rested 8 inches from the end of the trailer floor. The chemicals that required rearranging were in a powder form and stored in 40-pound bags. The bags were stacked approximately 4-feet high on pallets. Employee #1 used a pallet jack to rearrange the chemicals, moving the load from the front of the trailer toward the rear. The rearrangement of the pallets left little space for Employee #1 to exit the trailer, requiring him to squeeze between the pallets, or between the pallets and the wall of the trailer. Employee #1 attempted to exit the trailer by climbing on top of one of the mounds of chemicals, located near the rear of the trailer. As he reached the rear of the trailer, he lost his balance and fell. His left leg got caught in the 8-inch space between the lift gate and the floor of the trailer. Employee #1's body continued to fall while his left leg remained between the trailer and the gate. Employee #1 was hospitalized and treated for his injury.
