Investigation Summary
On July 14, 2005, Employee #1, a carpenter, was unloading tools and equipment from a job-made haulage box. The box was elevated 14 ft, to a second story balcony, on the forks of an industrial lift truck. The box was not secured on the forks, and when Employee #1 went into the box, it became unbalanced and fell to the ground. Employee #1 fell with the box and was injured when he impacted the ground and the contents inside the box fell on top of him. Employee #1 was hospitalized.
