Employee Amputates Finger Using Concrete Casting Machine

On May 26, 2005, Employee #1 was assigned to his usual and customary occupation as a machine operator. Employee #1's task was to operate Columbia Machine Company Model Number 10ACH11, Serial Number 662910AH10, concrete casting machine. The machine manufactures precast concrete boxes. While operating the machine, Employee #1 rested his right hand on the start button housing with his fifth finger extended. The finger was extended into an unguarded area of the machine where the head-stop rod and the foot-stop rod come together to create a pinch point. The tip of Employee #1's finger was crushed by the action of the two rods coming together. Employee #1 suffered an amputation but was not hospitalized.
