Employee receives injuries in vehicle collision

At approximately 1:30 p.m. on September 29, 2004, Employee #1 was operating a pickup truck in the container yard when he collided into three 4.33 ft diameter metal post bollards. He placed the vehicle in reversed and accelerated backwards. As he accelerated backwards, he struck a parked refrigerated-cargo trailer. The trailer rammed the cab of his pick-up truck, pinning the pickup truck seat to the undercarriage of the trailer. He sustained a humerus fracture and left lung puncture. Emergency medical personnel were summoned and arrived on the scene. They arrived and transported him to a medical center, where he was hospitalized for over 24 hours. The accident investigation determined that the vehicular collision was due to human error. The employer was not issued any accident related violation, but a Regulatory Citation for failure to notify the Division pursuant to the occurrence of a serious injury, T8 CCR sec. 342(a).
