Employee sustains fracture after falling from rolling-ladder

On January 27, 2003, Employee #1 was injured by a 12 ft high, rolling-ladder stand we was working on tipped over. Contributing factors in this accident were the use of the ladder stand on a sloped surface and the 4 to 1 height to width ratio of the ladder stand (allowable under Fed/OSHA rules but not Cal/OSHA rules, which require a 3 to 1 height to width ratio). The employer had failed to report this accident to Cal/OSHA, which learned of the accident nearly one year after it had occurred.
