Employee Is Injured in Fall from Aerial Manlift

On March 4, 2004, Employee #1 was in the basket of a JLG boom lift when the boom platform was struck by a U-Haul truck. Employee #1 was ejected from the basket and fell approximately 14 feet. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for multiple fractures. The accident investigation revealed that Employee #1 was not tied off and had not been trained on the use of the lift. Further, appropriate traffic controls were not in use.
