Employee killed when struck by truck

On December 4, 2003, Employee #1 from Environmental Structures, Incorporated, was struck by a subcontractor's truck when he was walking across a driving area of the U.S. Foodservice property. Employee #1 was engaged in a conversation on his cell phone at the time of the incident and apparently did not hear the back-up alarm of the truck as it was moving in a reverse mode. Employee #1 was killed.
