Employee dies in fall from manbasket

On October 8, 2004, Employee #1 was standing on a work platform, called a manbasket, which was set on the top tines of a forklift. The manbasket was not secured to the forklift. Being at an approximate height of 15 ft, Employee #1 stepped up onto the mid rail of the basket. The basket flipped sideways off of the tines of the forklift and Employee #1 jumped from the basket. He landed on his feet on top of another piece of equipment. Employee fell backwards onto a concrete floor and landed on his back and head. Employee #1 was transported to a hospital where he died a few hours after the accident.
