Employee's three fingers amputated by saw blade

On February 23, 2004, Employee #1 was using a table saw equipped with a dado blade. He was cutting a 1-in.-wide-by-0.5-in.-deep groove in a 1-in.-by-6-in.-by-48-in. piece of lumber. Employee #1's right hand came in contact with the saw blade and amputated his right thumb, index finger and middle finger. He was hospitalized.
