Investigation Summary
At approximately 5:00 p.m. on February 19, 2004, an employee and a coworker were positioning a precast concrete slab on the second story of a motel. The slab weighed approximately 4,500 pounds. The slab tilted while the workers were standing on top of it. The employee was killed when the concrete slab fell on top of him. The coworker was able to jump to safety.
