Investigation Summary
Two electric power generating station employees were opening a suction valve for a boiler circulating-water pump to bring the pump back on-line after it had been out of commission during repairs. A 68-kilogram section of the motor-operated valve actuator housing blew off, striking one of the workers in the head and upper torso. Emergency medical services transported the injured worker to a hospital, where he died of his injuries the following day.
