Investigation Summary
On June 29, 2004, Employee #1 was engaged in a minor plumbing repair job at the Muny Theatre in St. Louis, MO, when he apparently began to feel hot. He sat down in the front seat of his van and turned on the air conditioner. Employee #1 was later found there, unconscious, and was transported to the hospital. He died of an apparent heart attack.
