Investigation Summary
At approximately 1:30 p.m. on Friday, January 26, 2007, Employee #1 was working as a forklift operator in a furniture rental company warehouse. He was apparently retrieving furniture from the top rack at the time of the accident. He had used a high lift order picker (forklift) to reach the top rack. He then stepped off the order picker platform onto the furniture rack area at a height of 16 feet 2 inches. He fell from there onto a concrete surface. He sustained a head injury, and he was killed.
